What is Social Prescribing?

Many things affect patients’ health and wellbeing.  Many patients speak to us about feeling isolated or lonely, being stressed out by work or they have money or housing problems. Sometimes the stress of managing different long-term conditions can get people down.

That’s where social prescribing comes in and it starts with a conversation. It might be the conversation you’ve just had with your doctor, or with another person in the practice team who can then make a referral to our Link Worker who is there to listen to you and put you in touch with the people and activities that might help you to feel better.  The Link Worker will work with you to understand what matters to you to improve your physical and mental wellbeing, for example:

  • Befriending, counselling, and other support groups
  • Arts, gardening, creative activities and outdoors
  • Healthy lifestyle advice and physical activity
  • Meeting people or identifying and attending local groups and activities
  • Accessing specialist services and support.

How can I get support from a social prescribing link worker?

If you think that a referral to social prescribing might help you, please let your GP Practice know so they can put you in touch with our social prescriber or Alternatively complete the self-referral below.

Once you’ve been referred to the social prescribing service, one of the admin team will contact you, usually within two weeks. We sometimes have a short waiting list so please bear with us. Primarily we will support you over the phone at a time that’s convenient to you but if you need a face to face appointment we will do our best to accommodate this.
